MEDICAL SOCIAL WORKER - HOME HEALTH at Bluefield

We currently seek aPRNMasters Level Medical Social Workerto join ourTotal Home Health CareteaminBluefield, WV.

• Provides social work services in accordance with the physicians plan of treatment. Accepts only clients referred by the client care manager.

• Assists the physician and other members of the home health team to understand the significance of social and emotional factors related to the clients care.
• Assesses and reassesses social and emotional factors in order to help the client and family cope with problems in daily living and to help them follow medical recommendations to maximize the clients health condition.
• Counsels clients and families concerning the facilitation of the medical plan of treatment.
• Recognizes and utilizes community and family resources to assist the client in long-term plans for life in the community or to learn to live within the clients disability.
• Develops the plan of care with the case manager, physician, client and family to deal with personal, financial and environmental difficulties that may interfere with the health and wellness continuum. Participates in discharge planning.

 Requirements

• A masters level social work degree from a school of social work accredited or approved by the Council of Social Work Education
• Currently licensed to practice as a social worker in the state ofWV.
• Minimum of one yearHome Healthexperience preferred
• Valid Drivers License
• Clinical experience in varied multidisciplinary settings
• Ability to be open and flexible
